Crisp County High School Athletic Training 

GHSA regulations require students to have an annual Pre-participation Physical Exam (PPE) completed each year in order to participate in sports. It is now a 2 step process and instructions are listed below:

Follow the steps below:

Step 1:

A. (Incoming 9th graders or New Athletes to CCHS):

 B. (All Returning Athletes with an Dragon Fly Max Account already set up):

Click on the DragonFly Max "Update " Instruction Hurricane Logo Sheet Icon below on how to update your account for the uopcoming year.

Step 2:

Click on the GHSA logo and print out the Pre-Participation Physical Examination (download and take to physician to be completed).

 

Once you complete the online process in Step 1, then proceed to Step 2 - which is to have your child's physical exam completed by a physician and return to that physical examination form back to our Athletic Training Facility Office in Room 425 or upload the entire physical via photo to your childs DragonFly Max Account. CLICK ON THE ICONS BELOW FOR INSTRUCTIONS.
